NJWELL is a free, voluntary employee wellness program to help eligible members of the SHBP/SEHBP live a healthy lifestyle. Eligible members and their covered spouses or partners are rewarded for completing activities that encourage healthy behaviors.

No. Access to online tools and features are required to participate in NJWELL.

The health assessment will show as Complete once you respond to all questions. You’ll know that all questions were answered when you get your personal health score. If you don’t receive a score, the health assessment was not completed. Please make sure all health assessment questions are answered.
Your health screening will show as Complete once your results are submitted and processed by Quest Diagnostics (Quest). It may take up to 10 days for Quest to process your results depending on how you choose to complete the screening. Quest will email your results to you.

If you are eligible for the SHBP Retiree Wellness Program, the requirements for that program must be followed to enroll and to maintain enrollment.

If your Horizon member ID number has changed since the last time you signed in to the secure website, you must create a new Horizon user ID and password.
If you completed the health assessment and other online activities when you had a different Horizon member ID number, you must re-register and complete these activities again.
Remember, the value of the reward is based on your health plan at the time the reward is earned.

If you transfer from one participating SHBP or SEHBP employer to another, and you maintain SHBP/SEHBP health plan coverage, any NJWELL reward points earned by you or your spouse/partner will transfer with you, as long as you remain eligible for the program.
If you transfer to an employer that does not participate in the SHBP or SEHBP, you will no longer be eligible to participate in NJWELL. Rewards are issued to members who are covered by SHBP or SEHBP at the time the reward is earned.

Active full-time and part-time employees who are covered by an SHBP or SEHBP health plan, and their covered spouses/partners, are eligible to participate in NJWELL.
How to earn rewards as a DPC member
If you are a Direct Primary Care (DPC) member, you can earn your reward by having a well visit and follow-up visit with your provider (700 points) and taking the health assessment (100 points). The well visit and follow-up visit need to take place between November 1, 2023 and October 31, 2024. You will earn the 700 points for the two visits once the claims are processed. The 100 points for the Health Assessment will be credit upon completion of the Health Assessment.
